Soliciting some input - we'd like to be able to do some audit of people's practices. There are a couple of types of information we'd like to gather.
Provider info - what kind of provider are you/what type of licensing do you practice under/what board certifications do you have? What style of acupuncture do you primarily practice?
Patient feedback - patient demographic information (mainly age), what was/is the chief complaint? How long has the chief complaint been an issue? Chief complaint progress after x treatments?
We need a way to link de-identified patient responses to individual providers. We'd like to put this data in to a data warehouse where it can be used to answer questions and help individual providers improve as well as in the aggregate to have some objective data around acupuncture/East Asian Medicine's effectiveness.
We've talked about using paper forms, an app, or something like SurveyMonkey as a data gathering point. What we're looking for is some discussion around what solution might present the most effective way to gather the data we're interested in.

I don't have a solution BUT, how about each one (or two of us) tried a different method ie; monkey survey, forms, apps..... We can trial and essentially critically analyse the different methods for a month or so. Then, we can get together and collectively choose the best method.
